How do I upgrade from 1.2.0 to 1.2.1?
Do I run the install.php file, or just copy the files over the old ones?

No, you download the upgrade script from our download page. Put it inside the forum's main web directory and run it. It will upgrade your forum.

The manual upgrade procedure that I use, when update doesn't work for my windows box is... (YMMV)

Back up your ForumData directory and your htdocs directory before you try this -- Also note that it doesn't work for Database scheme changes AFAIK
download the tar.gz
unzip to forum_install directory
delete forum_install\install_data\sys\include\GLOBALS.php (so that your preferences are not over written)
copy all of the files from forum_install\install_data\sys\include over those in your ForumData directory's include directory
copy all of the files from forum_install\install_data\docs over those in your forums directory in your DOCUMENT_ROOT

If it fails restory your ForumData and htdocs from the backup you made.
