You've upgraded MySQL to 5.5. However, FUDforum versions before 3.0.3 is not compatible with MySQL 5.5.

Two possible solutions:
1. Upgrade FUDforum!!!
2. Apply the patch at http://fudforum.org/forum/index.php?t=msg&th=119924

Just so I'm clear, to upgrade from 3.0.3, we also have to upgrade our PHP, correct? We're running 5.1.6 now.

Yes, FUDforum 3.0.3 requires PHP 5.2.3 or higher.

I applied the patch (manually edit of the referenced PHP to append it to the end) and now when I run a consistency check, I only get a blank page.

Double check if you've correctly applied the patch. In the unlikely event of it still not working, you need to debug, undo or restore.
