Keeping CSS in a separate file makes a LOT of sense, since the browser is then able to cache this information once and not fetch it again. By embeding into pages you make each page larger for no reason.

CSS cannot be inside the FULL_ROOT, since each theme can have its own stylesheet file, therefor they need to be in separate directories.

For avatars I'd wager you best bet would be intoroduce AVATAR_DISK_DIR and AVATAR_WWW_DIR variables into GLOBAS.php and use the to allow each site to specify their own avatar storage placeholder.

Several reasons, #1 to see what those settings are. Suppose you need to do some manual SQL work and forgot the forum's settings and what not. As far as adjusting the settings you can do that prior to manual data dump to another server so that the forum can start working in the new location instantly.

The file is linked in just 2 places, WWW_ROOT_DISK and adm/, which allows the forum to know precisely where it is without hardcoding its path with a full path.
