I wonder whether email notifications are always send when a message is posted to a subscribed topic/forum, or whether this only happens when the user is either considered inactive or is logged out altogether ?

The e-mail notifications are only sent if the user had acted upon the last notification sent. This prevents pointless messages that would flood the user's mailbox in the event they are not there.
