Check your .htaccess file -- it probably contains a line that looks like this:
php_value auto_prepend_file "foo.php"
with the contents of the file foo.php resembling
echo "!<DOCTYPE HTML PUBLIC...";
echo "<HTML><HEAD>";
etc.
