my mistake... i have an other error when i test others issues...
i correct the img tag attribute end it's working :)
<img src='captcha.php'alt="imagem ausente"/>
the session is needing to make this verification: // verifica se o código captcha está correto
if($_SESSION['codigo'] == $_POST['codigo'])
finally solved :)
