When I try to start install.php the browser immediately shows:

Internal Server Error
The server encountered an internal error or misconfiguration and was unable to complete your request.

The error log shows: Premature end of script headers: php-script

The directory and install.php are chmodded to 777.
OS: Linux
PHP version 4.3.6
safe_mode off

Installation tried with:
FUDforum v2.6.8 Release Candidate #1 and
FUDforum v2.6.7 with same results in both cases.

On the first run of install.php I got the step 1 screen, but something happened when I tried to go to step 2. After that I removed all files in the directory and created install.php again (gunzip and tar on the server).

Sounds like a PHP error (possible PHP crash), I wish I could tell you more but I'd only be able to do so by being on the server where this is happening.

The solution to this problem was interesting. I changed the permission of install.php and the containing directory from 777 to 755 and that solved the problem!

I know that many will be surprised by this solution, but in some environments you are not allowed to open up completely CGI directories and scripts. I suddenly remembered this from a similar problem I ran into once with a Perl script.

I see, thanks for the update, I'll need to remember this solution for the future.
