Refers to the POSITION OF THE FORUM, not the message it in. Try adding a forum and set the insert position to 1st, you'll see that the new forum is inserted as 1st inside the category.

I thought, that this would be set by

Category & Forum Management ->

Insert position: Last/First

but now it's clear.
