when approving avatars que from the index page recieve this error:
Not Found
The requested URL /adm/admapprove_avatar.php was not found on this server.
Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.
Can approve avatar normally through the avatar manager in control panel.
Is this looking for "admavatarapr.php"?

Nope! I've been able to reproduce it on a 2.8.0 upgraded to 3.0.3 even after removing custom themes, as well as on a new install, without any added themes!
Steps followed:
Installed/upgraded 3.0.3:
Applied rev 5388 admuserapr.php and core.inc.
Applied rev 5387 admavatar.php
Applied rev 5384 groupmgr.tmpl
Applied rev 5382 err.inc.t
Applied rev 5380 root_index.php.t
Checks ok!
Register as new user using valid e-mail, and follow confirmation process.
Login as new user Upload custom avatar, log-out!
Login as admin, click avatar queue on index page and receive "not found error", does not reflect in error logs.
