Well, it's not picking up the latest "msg" file. Custom theme with old messages maybe?

Nope! I've been able to reproduce it on a 2.8.0 upgraded to 3.0.3 even after removing custom themes, as well as on a new install, without any added themes!
Steps followed:

Installed/upgraded 3.0.3:
Applied rev 5388 admuserapr.php and core.inc.
Applied rev 5387 admavatar.php
Applied rev 5384 groupmgr.tmpl
Applied rev 5382 err.inc.t
Applied rev 5380 root_index.php.t
Checks ok!

Register as new user using valid e-mail, and follow confirmation process.
Login as new user Upload custom avatar, log-out!

Login as admin, click avatar queue on index page and receive "not found error", does not reflect in error logs.

When I came back issues correctly and it worked like a champ, tried to install 3: 1 upgraded, 1 new and 1 new custom themes and it worked on all of them.
